Kondratiuk was born in Pinsk in Western Belarus in 1936. He graduated from the Lodz film school in 1963.

Kondratiuk made low-budget films, sometimes working with amateur actors. He won the Bronze Leopard Eye at the Locarno film festival in 1985 for his autobiographical film Cztery pory roku and won the special jury prize at the 1970 Karlovy Vary Festival for the drama Dziura w ziemi.

He was awarded three times at the Gdynia Film Festival, including the special jury prize in 1984 for Gwiezdny pył and the Silver Lion in 1985 for Cztery pory roku.

He is survived by his brother, film director Janusz Kondratiuk and his wife, actress Iga Cembrzynska.
